ADVANCED GRADUATE CERTIFICATE FORMS


FORM #1: "PERMISSION TO ENROLL" IN THE ADVANCED GRADUATE CERTIFICATE IN ART AND PHILOSOPHY

PROCEDURE

Step I: The student should consult with the Graduate Certificate Program Director for the Advanced Graduate Certificate in Art and Philosophy to determine eligibility for the certificate program.

Step II: Once eligibility is determined and approved, the graduate student should print out [in duplicate] the PDF form (link above) and obtain the signature [on each copy] of :

(a) the Student's Advisor in the primary degree Graduate Program and

(b) the Graduate Program Director in the primary degree department or program.

Step III: The signed forms should then be brought to the Graduate Certificate Program Director who will also sign the forms and send the form on to the Graduate School with a recommendation that the student be admitted to and enrolled in the Advanced Graduate Certificate Program in Art and Philosophy.

Step IV: If the student is an International Student, the Graduate School will also need to obtain the approval of the International Student Advisor. But this can be done when the Graduate Certificate Program Director submits the permission to enroll form.

Step V: The Graduate School representative will then approve/sign the form and notify the Registrar that the student's transcript should reflect the student's enrollment in the Advanced Graduate Certificate.

Step VI: The Graduate Certificate Program Director will be informed when the student is officially enrolled in the Certificate Program. The Graduate Certificate Program Director will notify the student that the enrollment in the Certificate Program has taken place. The Art and Philosophy Graduate Certificate will appear on the student's transcript.

Step VII: At least once a year, the student should submit the online Progress Form (see below) until all requirements have been completed.

Step VIII: Once all requirements have been completed, the student should submit the Completion Form (see below).

Step IX: The Graduate Certificate Program Director will notify the Graduate School with an official Completion Statement, informing the Graduate School that the student has completed all requirements for the Advanced Graduate Certificate in Art and Philosophy.

Step X: The Graduate School will notify the Registrar that the student has completed the Certificate Program and that the student should be awarded the Advanced Graduate Certificate.


FORM #2: "PROGRESS THROUGH THE GRADUATE CERTIFICATE PROGRAM"

The student should consult regularly with the Certificate Director to indicate progress through the Certificate Program.

At least once a year, the student should complete the online form indicating Progress in the Graduate Certificate Program.

FORM #3: "COMPLETION OF ALL REQUIREMENTS FOR THE ADVANCED GRADUATE CERTIFICATE PROGRAM"

Once the student has satisfied all requirements for the program, the form below should be submitted online, indicating each course offered for the Certificate Preogram with the corresponding grade and instructor(s) for each course. This form is sent to the Certificate Graduate Program Director.

Upon approval of completion of the Graduate Certificate Program, the Certificate Program Director will notify the Graduate School that the student has satisfied all requirements for the Advanced Graduate Certificate.

The Graduate School will review the Certificate Program Director's "Completion Statement" and, upon acceptance by the Graduate School, the Certificate Program Director will be informed that the student has been approved to receive the Advanced Graduate Certificate in Art and Philosophy.

At this point, the Graduate School will notify the Registrar that the Certificate Program has been completed. The transcript should then indicate that the student has completed the program and should receive the Advanced Graduate Certificate.
